چکیده

In volume 19 (2015) of Critical Care, Liu et al. [1] present a systematic review of risks and benefits of stress ulcer prophylaxis (SUP) in adult neurocritical care patients. A total of eight randomised controlled trials (RCTs) on SUP with proton pump inhibitors or histamine-2 receptor antagonists versus placebo or no prophylaxis in neurocritical care patients was assessed. The authors conclude that SUP is superior to placebo/no prophylaxis in reducing gastrointestinal (GI) bleeding and all-cause mortality, while not increasing the risk of nosocomial pneumonia [1]. We would like to thank the authors for highlighting an important topic with significant clinical equipoise; however, we are worried that biased results and conclusions are presented. First, the review holds methodological limitations, including the fact that no predefined sensitivity analysis with continuity correction in the zero-event trials was planned or performed, and importantly the risk of random errors using trial sequential analysis (TSA) was not assessed. Applying TSA to the three trials with lowest risk of bias [1] suggests that an estimated required information size of 2005 patients and 1790 patients are needed to

Stress ulcer prophylaxis versus placebo or no prophylaxis in adult hospitalised acutely ill patients—protocol for a systematic review with meta-analysis and trial sequential analysis

BACKGROUND Stress ulcer prophylaxis is considered standard of care in many critically ill patients in the intensive care unit (ICU). However, the quality of evidence supporting this has recently been questioned, and clinical equipoise exists. Whether there is overall benefit or harm of stress ulcer prophylaxis in adult hospitalised acutely ill patients is unknown. Accordingly, we aim to assess ...

Risks and benefits of stress ulcer prophylaxis in adult neurocritical care patients: a systematic review and meta-analysis of randomized controlled trials.

INTRODUCTION Neurocritical care patients are at high risk for stress-related upper gastrointestinal (UGI) bleeding. The aim of this meta-analysis was to evaluate the risks and benefits of stress ulcer prophylaxis (SUP) in this patient group. METHODS A systematic search of major electronic literature databases was conducted. Eligible studies were randomized controlled trials (RCTs) in which re...

The present guideline from the Danish Society of Intensive Care Medicine and the Danish Society of Anesthesiology and Intensive Care Medicine sums up current evidence and gives clinical recommendations for SUP in the ICU. The GRADE approach was u...

Background and purpose: Stress ulcer or stress related mucosal disease (SRMD) is an important cause of morbidity and mortality in intensive care units. No drug prophylaxis for SRMD increase the rate of morbidity and mortality. Unnecessary use of drugs results in adverse drug reactions and impose additional costs. The purpose of this study was to investigate the medications used for stress ulcer...

Stress ulcer prophylaxis in critically ill patients: review of the evidence.

Critically ill patients are at risk of developing stress ulcers in the upper digestive tract. Multiple risk factors have been associated with the development of this condition, with variable risk of association. Decades of research have suggested the benefit of using pharmacologic prophylaxis to reduce the incidence of clinically important upper gastrointestinal bleeding, with no reduction in o...
